How do I obtain copies of my medical records?

After your appointment, Unique Imaging will generate a CD with your images and arrange to send them to your physician. Once your physician has these images then you must contact the physician who can provide them to you upon request. Please contact our medical records department for information for a copy of your images. A charge may be issued to you if such request is made.

How do I pay for my appointment?

Upon your arrival at Unique Imaging, please provide a record of your insurance information, valid photo ID and physician referral. If your visit is due to an auto accident them please provide your auto insurance information containing you PIP/Auto claim number, date of incident, auto insurance card, contact name and telephone number of your insurance carrier along. Upon review, we may need to further verify your insurance.

If you elect to pay out of pocket, then please bring a valid debit/credit card with you as we do not accept checks.

How do I obtain pre-authorization?

Please call your insurance provider and see if you need pre-authorization first before coming to Unique Imaging. It is a good idea to also ask your insurance provider additional questions related to your coverage should you have additional payment queries.
